Job Summary:

The Hospice Admissions Nurse is responsible for coordinating and facilitating the admission process for patients entering hospice services. This role serves as a clinical liaison between referral sources, intake staff, and the interdisciplinary care team to ensure timely, accurate, and compassionate admission of patients.

 

The Admissions Nurse gathers and reviews clinical information, performs consults and eligibility assessments in collaboration with the hospice provider, and educates patients and families on hospice philosophy, services, and care expectations. They ensure all documentation meets regulatory and compliance standards while supporting a seamless transition into hospice care.

 

This role requires strong clinical judgement, effective communication and excellent organizational and time management skills. The Admissions RN must be adaptable and able to adjust to frequently changing schedules while maintaining a patient and family centered approach in a fast paced environment.

 

Supervisory Responsibilities:

  • None

 

Duties/Responsibilities:

  • Complete an initial assessment of patient and family to determine hospice needs; perform a comprehensive physical assessment and obtain history of current and past illnesses
  • Provide professional nursing care by utilizing all elements of the nursing process
  • Develop a plan of care that establishes goals based on nursing diagnosis and incorporates palliative nursing interventions
  • Initiate the plan of care and make necessary revisions as patient status and needs change
  • Counsel the patient and family in meeting nursing and related care needs
  • Provide health care instructions to patients and families in accordance with the assessment and established care plan
  • Administer medications and treatments as prescribed by the provider
  • Foster and maintain collaborative relationships with community partners, referral sources, and internal hospice team members to support continuity of care
  • Actively build, strengthen, and sustain community relationships through ongoing communication, education, and outreach to referral sources and healthcare partners, promoting hospice services and ensuring timely, appropriate patient referrals

 

Required Skills/Abilities:

  • Knowledge of appropriate HIPAA and confidentiality
  • Ability to accurately use the PAINAD scale
  • Maintain accurate documentation
  • Able to work as both a part of a team and independently
  • Highly organized and detail oriented
  • Excellent time management and multitasking skills.
  • Strong understanding of hospice eligibility guidelines and Medicare regulations
  • Knowledge of Microsoft products i.e. Excel, Word, etc.

 

Education and Experience:

  • Graduate of Accredited Nursing Program
  • Posses a valid Minnesota Registered Nurse (RN) License
  • Current CPR/BLS Certification
  • Minimum of 2 year nursing experience in hospice or palliative care

 

Physical Requirements:

  • Knowledge of appropriate HIPAA and confidentiality

  • Able to work as both a part of a team and independently
  • Possesses great organizational Skills
  • Excellent time management skills.
  • Valid driver’s license and auto insurance
  • Knowledge of Microsoft products i.e. Excel, Word, etc.
