Job Title: Certified Hospice Aid (CNA)

Reports to: CNA Supervisor & Director of   Nursing

 

Job Summary:

As a Certified Hospice Aid is responsible for providing personal care related to the daily living activities of hospice patients. Our patients may reside in facilities and or home environments.

 

Supervisory Responsibilities:

  • None

 

Duties/Responsibilities:

  • Provide personal care as directed in the patients personalized plan of care.
    • Tasks may include but are not limited to bathing, dressing, grooming, toileting, skin care, nail care, etc.
  • Perform lifts and transfers utilizing proper body mechanics and safety measures.
  • Assist with range of motion, positioning, exercises and ambulation.
  • Provide assistance with nutrition.
  • May perform light housekeeping duties when requested.
    • Task may include but are not limited to washing dishes, laundry, dusting, vacuuming, making beds and meal preparation
  • Ensure facility shower rooms are clean and tidy after performing patient care.
  • Communicate condition changes, concerns and general status updates to the RN Case Manager.
  • Ensure complete documentation of observations and cares performed in patient’s electronic medical chart after each visit.
  • Monitor and deliver patient supplies each visit.
  • Maintain a safe environment for all patients, reporting any unsafe conditions to the RN Case Manager, family and/or facility partner.
  • Communicate weekly schedule with RN Case Manager and CNA Supervisor.
  • Maintain communication with CNA Supervisor regarding supply needs.
  • Review patient care plans prior to each visit to ensure accuracy of care provided.
  • Communicate with RN Case Manager, facility and caregiver any changes to patient care plan.

 

Required Skills/Abilities:

  • Knowledge of appropriate HIPAA and confidentiality
  • Ability to accurately use the PAINAD scale
  • Maintain accurate documentation
  • Able to work as both a part of a team and independently

 

  • Possesses great organizational Skills
  • Excellent time management skills.
  • Valid drivers license and auto insurance
  • Knowledge of Microsoft products i.e. Excel, Word, etc.

 

Education and Experience:

  • Maintain a valid Nursing Assistant Certification
  • Minimum of 2 years CNA experience
  • One year hospice or long term care experience preferred

 

Physical Requirements:

  • Able to regularly lift and move up to 50lbs
  • Regularly operate and assist with physical and mechanical transfers
