Clinical Intake Admissions Nurse

Job Summary:

The Admission & Clinical Intake RN will be responsible for planning, organizing and directing hospice care with an emphasis on community education. They provide information about the initial examination and course of action for diagnosis and treatment for the purpose of coordination of direct services to meet the needs of patients and families within their homes and communities. Additionally, this position serves as a clinical liaison between the intake and admissions teams, providing clinical knowledge, skill and oversight in the day-to-day admissions process.

Duties/Responsibilities:

Admission RN

  • Complete an initial assessment of patient and family to determine hospice needs. Provides a complete physical assessment, history of current and previous illness
  • Provide professional nursing care by utilizing all elements of the nursing process
  • Develop a plan of care that establishes goals, based upon nursing diagnosis and incorporates palliative nursing actions
  • Initiate the plan of care and make necessary revisions as patient status and needs change
  • Counsel the patient and family in meeting nursing and related needs
  • Provide health care instructions to patient as appropriate per the assessment and care plan
  • Administer medications and treatments as prescribed by the provider
  • Foster and maintain relationships with community partners, hospice care teams, and referral sources 

Clinical Intake RN

  • Receives hospice inquiry via phone, website, in person or from our intake team or field staff
  • Collect information regarding patient’s current and past clinical history, presenting diagnoses, comorbidities, recent hospital activity and administrative information needed for potential hospice eligibility. Perform physical eligibility assessment as needed. Provide organized report to hospice physician to determine hospice eligibility.
  • Liaison for giving and receiving RN, SW and NP/MD reports from facilties, clinical teams and specialty clinics
  • Provide assessment of living situation and physical or psycho-social barriers to hospice enrollment or care
  • Make assessment of immediate DME needs
  • Determine urgency of admission
  • Coordinates admission date and time with patient, family and facility
  • Provide care navigation resources/support to patients/families/community partners

 

Required Skills/Abilities:

  • Knowledge of appropriate HIPAA and confidentiality
  • Ability to accurately use the PAINAD scale
  • Maintain accurate documentation
  • Able to work as both a part of a team and independently
  • Possesses great organizational skills
  • Excellent time management skills.
  • Valid driver’s license and auto insurance
  • Knowledge of Microsoft products i.e. Excel, Word, etc.

 

Education and Experience:

  • Graduate of Accredited Nursing Program
  • Posses a valid Minnesota Registered Nurse (RN) License
  • Current CPR/BLS Certification
  • Minimum of 1 year nursing experience in healthcare setting
  • One year hospice or long term care experience preferred
  • Willingness to obtain the certification in Hospice and Palliative Care within 2 years of employment.

 

Physical Requirements:

  • Able to regularly lift and move up to 50lbs
  • Regularly operate and assist with physical and mechanical transfers
  • May have prolonged periods of sitting at a desk and working on a computer
  • Personal protective equipment provided, including masks
