Minnesota Hospice provides care wherever the patient calls home including:

– Private home (house, condo or apartment)

– A friend’s or family member’s home

– Assisted living or memory care

– Skilled nursing facility

Your hospice care location depends on your support needs, and what’s realistic for the family. We will help you think it through.

 

What does hospice cost?

For most families, nothing. Hospice is covered under Medicare Part A (hospital insurance). The vast majority of our families have no out-of-pocket expense for hospice services under Medicare or Medicaid. Most private insurance plans also cover hospice services, though coverage varies by plan.

You’ll have a team that supports and guides, ensuring your loved ones feel comforted and cared for.

  

You’ll see the same familiar faces, not a different person every visit. Our team members live and work in the same communities they serve. They understand local resources, respond quickly, and build relationships with families.

 

Your care team may include:

  • Nurses for symptom management and regular visits

  • Home health aides for personal care support

  • Social workers for emotional support and resources

  • Spiritual care (if desired)

  • On-call nurse support 24/7 for urgent questions
