Job Summary:

The Community Liaison is responsible for driving business growth by generating new opportunities and expanding existing accounts within an assigned territory. This role emphasizes proactive, face-to-face engagement with healthcare providers, senior living communities, hospitals, and community organizations. The Community Liaison educates stakeholders on available services, builds strong relationships based on trust, and positions the organization as the preferred provider of care. Success in this role requires a strategic approach to territory management, a deep understanding of healthcare referral patterns, and a commitment to fostering long-term community partnerships.

Duties/Responsibilities:

  • Contribute to strategic planning by identifying potential referral opportunities to support continued growth
  • Prospect and develop new individual business opportunities within the assigned territory
  • Develop and execute a territory sales and marketing plan to achieve census and revenue goals
  • Assist the Director of Business Development in establishing referral projections and quarterly/yearly budgets
  • Communicate directly with the Director of Business Development regarding growth strategies, budget, internal/external issues, and customer service concerns
  • Build and maintain strong referral relationships with physicians (including specialty and rounding NPs), hospitals, discharge planners, case managers, social workers, senior living communities, and other community partners
  • Conduct in-person sales calls, services presentations, and patient/family consults as needed
  • Represent the organization at community events, networking meetings, and professional associations
  • Develop, oversee, assist, and/or market key programs, including Transitions, We Honor Vets, Hearts in Action and Education initiatives
  • Identify and qualify referral opportunities and assist with facilitating the referral and admission process
  • Collaborate closely with clinical, admissions, and operations teams to ensure smooth transitions of care and exceptional customer service
  • Track, document, and analyze all sales activity, referrals, and outcomes using Trella CRM, including monitoring progress, performance, competitor activity, and market trends with Trella Marketscapes and Sales Spotlights.
  • Reply promptly to correspondence with teams, families, patients, and referral partners
  • Maintain professional appearance, behavior, and demeanor at all times
  • Use respectful language when interacting with others
  • Establish and maintain effective working relationships
  • Handle interactions with patients, family members, team members, and referral partners with compassion, tact, and professionalism
  • Ensure all sales and marketing activities comply with healthcare regulations and ethical standards
  • Understand and adhere to laws and regulations regarding anti-kickback, lawful marketing activities (gifts and services), patient solicitation, and accurate marketing materials

 

Required Skills/Abilities:

  • Knowledge of CMS regulations and guidelines of hospice
  • Experienced in CRM systems
  • Competent in all Microsoft programs
  • Excellent oral and written communication skills
  • Outstanding organizational skills
  • Excellent interpersonal skills

 

Education and Experience:

  • Bachelors degree in business, marketing, healthcare administration, sales or nursing preferred.
  • 2-5 years of experience in healthcare sales, business development or community outreach, preferred.
  • Proven experience building and maintaining relationships.
  • Experience in territory management, sales planning and achieving growth targets.

 

Physical Requirements:

  • Able to regularly lift and move up to 20lbs
  • Must be able to safely operate a motor vehicle for extended periods of time, including frequent travel between patient homes, facilities, or work sites.
  • Personal protective equipment provided, including masks
