Hospice Nurse Practitioner Weekends

Job Summary:

The weekend Nurse Practitioner is primarily a home/office based position with occasional field needs. This role serves predominantly to support the weekend field staff with symptom management and admissions.

Supervisory Responsibilities:

  • None

 

Duties/Responsibilities:

Home/office based duties/responsibilities (approximately 75%):

  • Provide symptom management support through the use of pharmaceutical and other interventions
  • E-scribe controlled (CII-V) and non-controlled medications
  • Support field nursing staff via phone as needed for assessment, diagnosis, management, and/or evaluation of new or existing conditions relating to the patient’s terminal diagnosis
  • Conduct periodic medication reviews and provide recommendation to the care team abased upon safety, hospice philosophy and the goals of care

Field based duties/responsibilities (approximately 25%):

  • Conduct face-to-face exams as required by Medicae forhospice certification/re-certification and re-admission
  • Participate in care conferences either by phone or in person to support the patient, family, facility and hospice staff in coordination and goals of care
  • Perform in-person visits as needed for acute support of symptom management or change of condition when patient needs exceed the scope of the RN

 

Required Skills/Abilities:

  • Experience in assessment and treatment of basic acute care, including minor trauma, wounds, and infectious disease
  • Preferred experience in chronic diseasemenagement, palliative care, geriatric focused medicine, and/or symptom management
  • Knowledge and compliance of HIPAA and confidentiality
  • Excellent in both collaborative and autonomous work
  • Comfortability with various forms of technology including Mictorsoft products, e-subscribing applications, phone-based communication applications and EMRs

Education and Experience:

  • Master or Doctorate level education in advancedpractive nursing from an accredited university or program
  • Posess an APRN license in good standing through the State of Minnesota
  • Hold a board certification in Family Practice or Adult Geriatrics through a national accreditation agency (AANP or ANCC)
  • Hold or is qualified to obtain a DEA license

Physical Requirements:

  • Able to regularly lift and move up to 20lbs
  • Prolonged periods of sitting at a desk typing
