Job Summary:

The Weekend Nurse Supervisor provides clinical leadership, support, and operational oversight for all weekend hospice services. This role ensures high-quality, patient-centered care, timely response to patient and family needs, and effective coordination with the interdisciplinary team. The Weekend Nurse Supervisor serves as the primary point of contact for weekend staff.

 

Supervisory Responsibilities:

Under the direction of the Director of Nursing, the Weekend Nursing Supervisor is responsible for managing and supervising the following disciplines on the weekends: Registered Nurses, Licensed Practical Nurses

 

Duties/Responsibilities:

  • Co-facilitate quarterly weekend nurses meetings
  • Provide real time clinical guidance to weekend nursing staff
  • Support triage needs, urgent visits, symptom management and care planning
  • Ensure that regulatory standards and Minnesota Hospice policies and procedures are followed
  • Serve as the weekend resource and escalation point for all field staff
  • Coordinate & oversee weekend scheduling needs, visit assignments and staffing adjustments
  • Facilitate communication between weekday and weekend teams as needed
  • Maintain situational awareness of all weekend census activity, admissions and significant changes in condition
  • Ensure accurate and timely documentation of supervisory actions as required
  • Collaborate with oncall providers and leadership as necessary
  • Monitor for quality and safey concerns and escalate appropriately
  • Support performance improvement initiatives

 

Required Skills/Abilities:

  • Knowledge of CMS regulations and guidelines of hospice
  • Experienced in EMR systems
  • Competent in all Microsoft programs
  • Outstanding organizational skills

 

Education and Experience:

  • Current Minnesota nursing license
  • Minimum of a Bachelors degree in Nursing
  • Minimum of two (2) years direct support hospice experience preferred
  • Willingness to obtain a Certification in Hospice and Palliative Care (CHPN) within two (2) years
  • Strong clinical assessment, critical thinking and problem solving experience
  • Ability to lead calmly and effectively during urgent or emotionally challenging situations

 

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer
  • Must be able to lift 15 pounds at times
