Job Summary:
The Hospice Support Registered Nurse provides compassionate, hands-on care to hospice patients, assisting with daily living activities and personal care. This role is essential in delivering high-quality, patient-centered support while collaborating closely with the broader hospice care team to ensure comfort, dignity, and holistic care for each patient.
Supervisory Responsibilities:
Duties/Responsibilities:
- Participate as a member of the Minnesota Hospice care team and attend IDT meetings, educational in-services, and staff meetings as needed.
- Maintain communication with providers and other staff members to coordinate optimal care for the patient, family, and facilities.
- Provide education regarding medication instructions to patients, families, and facilities.
- Coordinate and communicate medication needs, orders, and delivery with the Nurse Practitioner and/or RN Case Manager.
- Coordinate equipment and supply needs with the Assistant Director of Nursing.
- Coordinate the delivery of equipment and supplies with patients, families, and facilities.
- Document all PRN visits, phone calls, and client interactions in the electronic health record within 72 hours.
- Implement care to achieve outcomes related to comfort and safe dying, self-determined life closure, and effective grieving for the patient, family, and facility.
- Maintain communication with facilities and families through the hospice binder, calendar, and visit record note entries at each visit.
- Communicate changes in the patient’s condition to the RN Case Manager and Director of Nursing.
- Administer medications and treatments as ordered by providers.
- Recognize clients’ psychosocial needs and report to the RN Case Manager, Social Work, and/or Chaplain as appropriate.
- Perform clinical tasks such as enemas, wound care, catheter care, ear lavage, and other procedures as needed.
- Provide emotional and psychosocial support to patients and families when needed.
- Foster and maintain positive relationships with community partners to enhance patient care and promote Minnesota Hospice services.
Required Skills/Abilities:
- Knowledge of appropriate HIPAA and confidentiality
- Ability to accurately use the PAINAD scale
- Maintain accurate documentation
- Able to work as both a part of a team and independently
- Possesses great organizational Skills
- Excellent time management skills.
- Valid driver’s license and auto insurance
- Knowledge of Microsoft products i.e. Excel, Word, etc.
Education and Experience:
- Graduate of Accredited Nursing Program
- Posses a valid Minnesota Registered Nurse (RN) License
- Current CPR/BLS Certification
- Minimum of 1 year nursing experience in healthcare setting
- One year hospice or long term care experience preferred
- Willingness to obtain the certification in Hospice and Palliative Care within 2 years of employment.
Physical Requirements:
- Able to regularly lift and move up to 50lbs
- Regularly operate and assist with physical and mechanical transfers
- May have prolonged periods of sitting at a desk and working on a computer
- Personal protective equipment provided, including masks
