Personal Information Collected.

Information You Choose to Provide to Minnesota Hospice.

Minnesota Hospice collects Personal Information that you choose to provide to Minnesota Hospice when you (1) use the Site; (2) contact Minnesota Hospice through the Site, such as via an email communication or contact form.

Notwithstanding the foregoing, Minnesota Hospice may aggregate any or all of your Personal Information for any purpose provided such Personal Information is de-identified such that it is no longer identifiable as your Personal Information.

Minnesota Hospice understands your interest in keeping Personal Information secure. Minnesota Hospice makes every effort to protect the security of your Personal Information. Minnesota Hospice has implemented technology to reduce the risk of unauthorized access, use or disclosure or the accidental loss or destruction of your Personal Information.

NOT WITHSTANDING THE FOREGOING, MINNESOTA HOSPICE DOES NOT WARRANT OR REPRESENT, AND EXPRESSLY DISCLAIMS ANY SUCH WARRANTY OR REPRESENTATION, THAT PERSONAL INFORMATION WILL BE PROTECTED AGAINST UNAUTHORIZED ACCESS, MISUSE, LOSS OR ALTERATION. YOU ACKNOWLEDGE, BY USING THIS SITE, THAT NO SECURITY SAFEGUARDS OR METHOD OF TRANSMISSION OVER THE INTERNET ARE COMPLETELY SECURE. UNFORTUNATELY, THE TRANSMISSION OF INFORMATION VIA THE INTERNET IS NOT COMPLETELY SECURE. ALTHOUGH WE DO OUR BEST TO PROTECT YOUR PERSONAL INFORMATION, WE CANNOT GUARANTEE THE SECURITY OF YOUR PERSONAL INFORMATION TRANSMITTED TO OUR SITES. Children

Minnesota Hospice does not intend that it’s Site be used by children under the age of 18. Minnesota Hospice does not knowingly collect Personal Information from children under the age of 18. If Minnesota Hospice becomes aware that a user under the age of 18 has submitted Personal Information to Minnesota Hospice via the Site, Minnesota Hospice will use commercially reasonable efforts to delete and expunge any Personal Information related to such child from Minnesota Hospice’s records.

Occasionally, at our discretion, we may link to third party sites or content on the Site. We may do this for the convenience of you and other users of the Site, but we do not have control over the operation of these third-party websites. Opt-Outs And Revisions To Your Personal Information

If you do not want to receive email communications from Minnesota Hospice, you may contact us via an email at the email address set forth in the Notice provision below with an opt-out request that specifies the email address at which you do not wish to receive email communications from Minnesota Hospice. Minnesota Hospice will make a good faith effort to honor all such opt-out requests. Please keep in mind, however, that if subsequent to submitting an opt-out request, you re-submit your email address again to Minnesota Hospice, you may begin to receive email communications again until you submit a new opt-out request. If you wish to revise Personal Information that you have provided to Minnesota Hospice, please send an email communication to the email address set forth in the Notice provision below detailing the Personal Information to be changed and the change to be made. Minnesota Hospice will make a good faith effort to ensure those changes are made.

Sale Or Transfer Of Information

In the event of a sale, merger, dissolution, liquidation of Minnesota Hospice or the sale of all or substantially all of the assets of Minnesota Hospice, any or all of the Personal Information and Use Data collected about you through your use of the Site may be sold, transferred or assigned to a third party in connection with such sale, merger, dissolution, liquidation of Minnesota Hospice or the sale of all or substantially all of the assets of Minnesota Hospice.
